When Do I Complete This eForm?

If you are an F-1 or J-1 CSU student who will be conducting research abroad or participating in a Study Abroad program, you are required to complete this eForm prior to departing the US. If you are eligible, an approved Out of Country request allows us to keep your immigration record active while you are outside of the U.S.

*Please note: Personal reasons do not qualify for the Out of Country approval. Meet with an ISSS advisor to discuss your options if you will be outside of the U.S. for personal reasons. 

Do not submit this form for authorization for travel during a school break. Instead, please submit a travel signature request here.

 Student Instructions for Completing This eForm

  1. Click the link above to access the eForm. Submit the eForm at least one month prior to your departure.
  2. After you submit your eForm, your Academic Advisor or Education Abroad advisor will receive an email to complete their portion of the eForm.
  3. Once your advisor has reviewed and completed the eForm, an ISSS advisor will review your request to determine eligibility for your Out of Country status. We will follow up with you if we have concerns about approval or need additional documentation to proceed.
  4. If your request is approved, we will email you an updated I-20 with your Out of Country status added.

Please note: Depending on the time of academic year, it may take up to 7-10 business days for our office to process your request.

CSU Health Insurance during your Out of Country Period

F-1 students on thesis/dissertation can request to waive their CSU Health Insurance in the Out of Country eForm.

F-1 students participating in a Study Abroad program are required to remain enrolled in the CSU Health Insurance Plan.

J-1 students must be enrolled in the CSU Student Health Insurance Plan as long as their J-1 record is active, unless they show alternate health insurance coverage that meets J-1 minimum requirements. Please email ISSS if you have questions about insurance.

Additional Student Information

  • If you will not be maintaining a U.S. address in your absence, please update your RAMweb account with the Office of International Programs address: 1024 Campus Delivery, Fort Collins, CO 80523-1024.
  • If you will not be enrolled in a full course of study during your time outside of the U.S., you must have an approved Reduced Credit Load on file before your departure.

Academic Advisor Instructions for the eForm

After the student submits the eForm, you, the academic advisor or Study Abroad advisor, will receive an email from ISSS with the subject line: “Action Needed: Out of Country Authorization Request for International Student.” Click on the link in the email to access the eForm.

  1. If you would like to see the information provided by the student in their section of the eForm, the links to their eForm are at the top of your eForm page.
  2. If your student is conducting research abroad, you will need to briefly describe the research topic and justify why the research needs to take place abroad. You will also need to verify the student’s enrollment in Continuous Registration or at least 1 credit while they are abroad. Please note that a student cannot be solely writing their thesis/dissertation abroad. They must be engaged in data-collection/research that requires them to be outside of the U.S.
  3. If your student is participating in a CSU Study Abroad program, you will need to verify the dates and location of their program and verify their upcoming participation.
  4. When you click “Submit,” your portion of the eForm will be automatically forwarded to ISSS for final review.
